§ 6.08.040. Vaccination certificate

There shall be provided by the poundmaster certificates in triplicate form together with corresponding tags for distribution to persons legally authorized, spaces for the following information and for such other information as may be required to be filled in by persons legally authorized to vaccinate dogs or cats. These certificates shall provide as follows:

A. The name and address of the owner or harborer of the inoculated dog or cat;

B. Date of inoculation;

C. Year and series number of the dog tag or cat tag;

D. Bread, age, color and sex of the inoculated dog or cat. The tag shall be of a durable material suitable to be attached to the collar or harness of the inoculated dog or cat. Such tag will state the year for which issued and the series number or certificate of the tag. Such tags and certificates will be prepared and distributed without charge by the poundmaster to the persons legally authorized to vaccinate dogs or cats.
